At Morningside Animal Hospital, our boarding services are designed to provide a safe, comfortable, and caring environment while you’re away. The boarding consent form allows us to understand your pet’s needs and ensure they receive the best possible care during their stay.

About This Form

The boarding consent form provides important information about your pet’s health, routine, and care preferences while staying with us. This form may include:

  • Owner and emergency contact information
  • Pet details and medical history
  • Feeding instructions and dietary needs
  • Medications and special care requirements
  • Authorization for treatment if needed
  • Vaccination confirmation

All boarding pets must be up to date on required vaccinations to ensure the safety of all guests. Providing accurate and complete information helps our team create a comfortable and stress-free experience for your pet.

If your pet requires medication or special accommodations, please include detailed instructions so we can follow their routine as closely as possible.

Feeding Instructions

Dogs staying more than 3 nights will receive a complimentary bath. Additional services are available at your request:

Dogs are required to be current on the following vaccines: Rabies, Da2pp, and Bordetella, and a current fecal (negative).

Cats are required to be current on the following vaccines: Rabies, FVRCP. Feline Leukemia status must be known, or the vaccine must be current with a negative fecal. If we do not have confirmation of these vaccines, they will be administered at your expense. This is for the health and protection of all pets in the hospital.

  • All pets must be free of internal and external parasites upon admittance. An intestinal parasite exam must be current within the last year.
  • Reasonable precautions will be used against injury, escape, or death of your pet. The hospital and staff will not be held liable for problems that develop, provided reasonable care and precautions are followed.
  • I understand that any problems that develop with my pet will be treated as deemed best by the veterinarian(s), and I assume full responsibility for the treatment expenses involved.
  • Payment is due in full at the time of pick up. If the pet is not called for within 5 days after the time specified for return and if the doctor is not notified in writing of an alternate date within the 5-day period, the animal will be considered abandoned and may be relocated as the doctor sees fit. It is understood that this does not relieve me from paying for all costs of your services and use of your hospital, including the cost of boarding, collections, and attorney fees. If any delinquent balances remain, I understand that a 1.5% monthly finance charge will be accumulated.
