At Morningside Animal Hospital, our Dental Consent Form allows you to authorize dental procedures for your pet, including cleanings, diagnostics, and any necessary extractions.

About This Form

This form gives our veterinary team permission to perform dental services under anesthesia.

It includes:

  • Authorization for dental procedures and potential extractions
  • Consent for recommended pre-anesthetic testing
  • Optional services such as EKG monitoring or microchipping
  • Acknowledgment of risks associated with anesthesia and surgery

Important Information Before Your Pet’s Procedure

To ensure your pet’s safety, please review the following:

  • Pre-anesthetic bloodwork is strongly recommended to assess your pet’s health before anesthesia
  • Vaccinations must be up to date prior to the procedure
  • Anesthesia carries inherent risks, which will be discussed with you in advance

If any concerns arise during the procedure and we cannot reach you, our veterinarians will act in your pet’s best interest.

Please complete the Dental Consent Form prior to your pet’s appointment to help us prepare.

As the Owner/Agent of the above pet, I hereby give my consent to Morningside Animal Hospital and its staff to perform the following surgical procedures:

I understand that my pet must be up to date on the Rabies vaccine and DHPP/FVRCP vaccine. I also understand that if my pet is not up to date on these vaccinations, they will be administered at my expense.

Morningside Animal Hospital will use all reasonable precautions against injury, escape, or death of my pet. I understand that anesthesia and surgery always involve some risk to my pet, and agree to hold Morningside Animal Hospital and its staff or agents harmless, in the absence of negligence, in connection with these procedures. I acknowledge that no guarantee or assurance has been made to me as to the results that may be obtained. If complications arise and I cannot be immediately contacted at the above-listed phone number, the veterinarians at Morningside Animal Hospital are directed to make the decision they deem best for my pet.

All charges, including boarding costs, shall be paid upon release from the hospital. If the pet is not called for within 5 days after the time specified for return and if the doctor is not notified in writing of an alternate date within the 5-day period, the animal will be considered abandoned and may be relocated as the doctor sees fit. It is understood that this does not relieve me from paying for all costs of your services and use of your hospital including the cost of boarding, collections, and attorney fees. If any delinquent balances remain, I understand that a 1.5% monthly finance charge will be accumulated.
