What You’ll Need to Know

Before submitting the form, please review key policies:

  • Respectful Environment: We maintain a safe and respectful space for all clients and staff
  • Appointments & No-Shows: 24-hour notice is requested; repeated missed appointments may require a deposit
  • Late Arrivals: Arrivals more than 15 minutes late may need to be rescheduled
  • Medication Requirements: A current exam is required for prescription approvals
  • Payment Policy: Payment is due at the time of service

Our Hospital Policies

To help us provide the best care for your pet and maintain a positive experience for all clients and staff, please review and initial the following policies.

Respectful Environment

Our hospital is committed to maintaining a respectful and safe environment for our team and our clients. Violence, threats, or verbal abuse towards our staff will not be tolerated and may result in termination of services at our hospital. If you have concerns or questions, please feel free to speak with our office manager.

Appointment Changes & No-Shows

If you need to reschedule your appointment, we kindly ask that you provide at least 24 hours’ notice whenever possible. Same-day cancellations or missed appointments will be recorded as a no-show. After three no-shows, a deposit may be required to schedule future appointments. We understand emergencies happen, and situations may be reviewed by the office manager when appropriate.

Late Arrivals

If you arrive more than 15 minutes late for your scheduled appointment, we may need to reschedule so we can remain on schedule for other patients.

Annual Exams for Medications

Florida law requires that a current physical examination within the past year be on file in order for veterinarians to prescribe medications. If your pet’s exam is not current, we will need to schedule an exam before approving medication requests.

Payment Policy

We do not offer billing. Payment is due at the time services are provided.
For your convenience, we accept CareCredit, Cherry, all major credit cards, cash, and personal checks (after an account has been established).

Financial Responsibility

I certify that I am the person responsible for authorizing and paying for all medical procedures and expenses for the pet(s) listed on my account.
