About This Form

The drop-off form provides authorization for our veterinary team to examine and treat your pet while they are in our care.

This form includes:

  • Owner and contact information
  • Pet details (species, breed, age, etc.)
  • Requested services or procedures
  • Authorization for additional care if needed

It also confirms your understanding that treatment plans may be adjusted if necessary based on your pet’s condition.

Important Information

By completing this form, you acknowledge:

  • Procedures may change based on your pet’s needs
  • Results cannot be guaranteed
  • You are responsible for all services provided
  • Payment is due at the time of discharge

Our team takes every precaution to ensure your pet’s safety while in our care.

Please complete and submit the drop-off form prior to your pet’s visit. This helps us prepare and provide efficient care.

I understand that during the performance of these procedures, unforeseen conditions may be revealed that necessitate an extension or variance in the procedure(s) set forth above.

I expect Morningside Animal Hospital to use reasonable care and judgment in performing the procedure(s). The nature of the procedure(s) and the risks involved have been explained to me, and I realize results cannot be guaranteed. I am also aware that unforeseen events resulting from the procedure(s) will not relieve me from any obligation to all costs incurred regarding this pet.

Morningside Animal Hospital will use all reasonable precautions against injury, escape, or death of my pet, but will not be held liable or responsible in any manner, as it is thoroughly understood that I assume all risks.

All charges, including boarding costs, shall be paid upon release from the hospital. If the pet is not called for within 5 days after the time specified for return and if the doctor is not notified in writing of an alternate date within the 5-day period, the animal will be considered abandoned and may be relocated as the doctor sees fit. It is understood that this does not relieve me from paying for all costs of your services and use of your hospital, including the cost of boarding, collections, and attorney fees. If any delinquent balances remain, I understand that a 1.5% monthly finance charge will be accumulated.
