About This Form

The euthanasia consent form provides authorization for our veterinary team to proceed with humane end-of-life care. It also includes important information about your pet and your decisions regarding aftercare.

This form will ask for:

  • Owner and contact information
  • Pet details (name, breed, age, etc.)
  • Authorization for euthanasia
  • Confirmation regarding health and safety considerations
  • Selection of aftercare and cremation options

You will also be asked to choose how you would like your pet’s remains handled, including private or communal cremation options. Please take your time when filling out this form. If you have any questions or need guidance, our team is here to support you every step of the way.

I, the undersigned, do hereby certify that I am the owner (duly authorized agent for the owner) of the animal described above; that I do hereby give Morningside Animal Hospital, their agents, servants, and representatives full and complete authority to euthanize the said animal in whatever manner the said Doctor, his agents, servants, or representatives shall deem fit.

I do hereby, and by these presents, forever release the said Doctor, his agents, servants, or representatives from any and all liability for so euthanizing the said animal.

I do also certify that the said animal has not bitten any person or animal during the last ten (10) days, and to the best of my knowledge has not been exposed to rabies.

** Please note that after 60 days any remains left unclaimed will be returned to Haisley Cremation Services .**
